WINTER INDOOR SOCCER REGISTRATION IS NOW OPEN!

THERE ARE TWO WAYS TO REGISTER:
 
1. Online Registration with mail in payment . 
You will receive a confirmation code which should be included in the memo section of your check so we can accurately record your payment - also include player(s) name(s)   All payments must be mailed to LVSC BOX 433, STANHOPE, NJ 07874 - registration is not complete until we receive your check.

2. Print registration form, complete form (PLEASE PRINT) and mail form and check to LVSC BOX 433, STANHOPE, NJ 07874.
 
REGISTRATIONS MUST BE RECEIVED BY DECEMBER 17, 2011 TO ENSURE WINTER PARTICIPATION AND TO AVOID A $20.00 LATE FEE
